Perimenopause fatigue: causes, red flags and what to track

Perimenopause can contribute to fatigue, but anaemia, thyroid disease, sleep apnoea, mood, infection and medicines also matter. Build a seven-day record.

Fatigue can be related without being explained

Sleep disruption and night sweats can make daytime fatigue worse during the menopause transition. But the NHS fatigue guidance also lists anaemia, thyroid disease, diabetes, stress, depression, illnesses and medicines. “Hormonal fatigue” should not end the assessment.

Clarify whether you feel sleepy, physically weak, mentally depleted or unable to recover after activity. Those experiences can overlap, but they lead to different questions.

Patterns worth discussing

  • Snoring, gasping or morning headaches: possible sleep apnoea.
  • Breathlessness, palpitations or pale skin: anaemia is one possibility.
  • Weight change, heat or cold intolerance, tremor: thyroid or other conditions may need consideration.
  • Low mood, anxiety or loss of interest: mental health deserves direct care.
  • Prolonged worsening after exertion: describe recovery rather than pushing through it.

WhenWhat to noteSimple rating
On wakingSleep length, awakenings, flushes, snoringRested 1–5
Mid-morningEnergy, focus, caffeineEnergy 1–5
After mealsMeal timing and sleepinessEnergy 1–5
After activityType, duration and recoveryEffort 1–5

A record builds a timeline; it does not diagnose iron deficiency, thyroid disease or sleep apnoea.

Accessing care in Spain

Start with your local centro de salud or médico/a de familia, or a private GP if that is how you access care. Bring a medicine list, bleeding history if relevant, the main functional impact and your short log. Use local urgent-care services or 112 for emergencies.

Frequently asked questions

Can perimenopause make you feel exhausted?

It may contribute through disrupted sleep, flushes and mood changes, but persistent exhaustion has many possible causes. Anaemia, thyroid disease, sleep apnoea, infection, medicines and depression are among the alternatives worth considering.

Which vitamins help menopause fatigue?

There is no universal fatigue vitamin. Iron, B12 or other supplements are appropriate only when a need is identified. Taking them blindly can be ineffective, cause side effects or delay assessment.

How long does perimenopause fatigue last?

There is no fixed duration. It depends on the cause and on factors such as sleep, symptoms, workload, medicines and health conditions. Seek care if fatigue lasts for weeks, worsens or limits daily life.

Where should I seek care in Spain?

You can start with your centro de salud or médico/a de familia, or use a private GP if that is how you access care. Urgent symptoms should go through local urgent-care services or 112.
